Access my medical records

Your medical records are held on paper and on computer in this practice. Anonymised data may be collected by Government Departments, please telephone 0300 456 3531 for further information. Your private medical details will not be shared with your family, unless you request us to do so.

All practice staff sign a confidentiality agreement before starting work. Information held on the computer is safeguarded by the Data Protection Act.

If you would like us to share information from your records with a third party- such as an insurance company- we will need your written consent before any disclosure can take place.

To access your records online- 

 

 

ONLINE ACCESS TO BOOKING/ CANCELLING APPOINTMENTS; ORDERING REPEAT PRESCRIPTION & YOUR MEDICAL RECORDS

 

If you are aged 16 years or over, we can set you up with online access to-

·        appointment booking/cancellations;

·        ordering repeat prescription and

·        your medical records.

 

PLEASE NOTE THAT YOU ARE CURRENTLY UNABLE TO BOOK/ CANCEL APPOINTMENTS ONLINE, THIS FACILITY HAS BEEN SWITCHED OFF DURING THE PANDEMIC AS ALL APPOINTMENTS ARE BEING TRIAGED.

Once the form is completed, please bring this into the Surgery along with two forms of ID- one form of photo ID, such as your passport or driving licence and one further form of ID issued within the last 3 months with proof of your address, for example a utility bill. If possible, please bring this in during our quieter times of 12pm-2pm or 5pm-6.30pm.

 

Please ensure that you follow the current COVID guidance and observe any social distancing/ face covering rules.

 

Once we have set your account up, you will receive an email with further instructions. 

 

 

 

If you would like a printed copy of your medical record, please ask at reception. Your copy will be available within 1 month (or 2 months if particularly complicated).
