Register with the practice
To register as an NHS patient you should be a resident of the UK and live within the Practice boundary area. Registration is easiest if you bring your NHS medical card to the practice. If you do not have this you will be asked to complete a GMS1 form which can be downloaded by clicking here (new babies –form FP58). You will also be given the following: Practice booklet and Medical questionnaire – to complete. You will be invited into the surgery for a registration check, usually with the Health Care Assistant.
GP registration
Anyone can register without needing proof of ID, address or immigration status, similar to that on the NHS.uk webpage. This is not a legal requirement and can often be the reason why people do not wish to register.
