Dunchurch Surgery patients can now use our online form system to submit Change of Address notifications.

----------------------------------------------------------------------------------------------------Following a Government announcement on 29th November, the NHS is developing plans to extend the Covid-19 booster vaccination programme to include everyone aged 18 and over.

12 to 15 year olds will also be offered a second dose of the vaccine and an additional booster will be available for people who are immunosuppressed.

 

People will become eligible for their booster three months after their second dose (reduced from six months).

For now, all you need to do is wait to be contacted by the NHS. Please help us to help all patients by not contacting your GP practice about your booster or attending vaccination sites.

 

If you are over 40 or a health and social care worker, and are eligible for your booster, go online to NHS website or call 119 to book your appointment. Visit HCP website to see all the places you can go for a walk-in appointment.

Covid 19 vaccinations update

 The practice strongly recommends vaccination against Covid-19. 

We have worked in collaboration with the other Rugby GP practices

(Rugby Health Ltd, Primary Care Network)

to administer vaccines at:

 Locke House, The Railings, Woodside Park, Rugby, CV21 2AW.

How to access your COVID-19 vaccination status

We are unable to issue this certificate at the practice.

Proof of your vaccination status will be available on the NHSapp.


The NHSapp can be downloaded from 
https://apps.apple.com/gb/app/nhs-app/id1388411277 for iPhone or https://play.google.com/store/apps/details?id=com.nhs.online.nhsonline for android.

Alternatively you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.

 

Do not contact your GP surgery about your COVID-19 vaccination status. GPs cannot provide letters showing your COVID-19 vaccination status.
